If you wish to Self-Exclude you can do so by signing up to the voluntarily exclude access to your account by signing up to the New York Self-Exclusion program. The duration for which you participate in this program is 1 year, 3 years, 5 years or a lifetime. You can sign up by completing the relevant form located on Regulator's website here. Final, notarized forms with photos may be submitted by mail to:

Alternatively, you may complete the forms at any of the licensed gaming facilities across the state. All requests must include a photo. Photos must be at least 2 x 2 inches and no larger than 4 x 6 inches. Photos must be recent (within six months) and display your full face from the neck up. Tinted glasses, hats and headwear are not permitted. If this self-exclusion request is being completed at any of the properties listed on the self-exclusion form, the photograph must be taken by a member of that property's security department.
A persons, upon the expiration of the selected term of voluntary exclusion, will have their playing privileges automatically reinstated, except in the case of lifetime self-exclusions.
Please bear in mind the following regarding Self-Exclusion:
• You will need to present government-issued photo-identification for the application process.
• Self-excluding from one brand on the Bally’s Network, will automatically prevent you from registering a new account or continuing to operate an already existing account on any other website managed by Bally’s.
• Upon self-exclusions being applied, any remaining balances on your account will be sent to you via check.
• Accounts closed for self-exclusion cannot be reopened for any reason until the self- exclusion period has expired. Upon expiry of the self-exclusion period, patrons may request to reopen a suspended account by contacting our 24/7 Customer Support Team.
