If you would like to self-exclude and take a permanent break from the platform, you must enroll with the Arizona Self-Exclusion program. You can self-exclude for 1, 5, or 10 years. Once you self-exclude, it will not be possible to reopen your account under any circumstances while you are self-excluded.
You can do this by completing the form located here. After the form is completed, you will need to have it notarized and then mail the form, along with a minimum 2" x 3" original color photo of yourself, which clearly shows your head and shoulders to the addressed listed below. A copy of a driver's license will not be accepted.
Please note: Your self-exclusion request will not be processed without proper notarization and a current photo. The photo may also be submitted electronically by emailing contact@problemgambling.az.gov. An appointment is required to ensure a notary is available.
